Health & Welfare Benefits

Medical - Vision - Dental - Life - Disability - Reimbursement Accounts

At Salient Surgical Technologies , Inc. we’re committed to our total strategy of offering comprehensive benefits, competitive pay, professional development, and a positive work environment. An integral part of this commitment is our comprehensive benefits program. Designed to meet the needs of a diverse workforce, this program lets you customize your benefits to meet your individual needs.

Eligibility: Full-time employees (more than 30 hours per week) and eligible dependents can participate on their date of hire.

 

Medical Insurance 

Choosing a medical plan for yourself and your family is a personal decision based on your specific needs. This is why Salient Surgical Technologies, Inc. provides a choice of several different medical plans based on what state you reside. These plans offer comprehensive health care services with varying levels of benefits. You can select the plan that best meets the needs of you and your family.

All medical plans provide vision care, prescription drug benefits, and dental benefits. They promote wellness as part of their benefits. The main differences between these plans are the deductibles, co-payments, co-insurance amounts, out-of-pocket maximums, health care delivery systems, and plan costs.

Medical plans available for your selection: Harvard Pilgrim Health Care-MA, NH, ME First Health-MD and NC Private Healthcare Systems (PHCS)-All other states

Prescription Drugs

When you enroll in a medical plan, you will automatically receive prescription drug benefits. All medical plans offer the same prescription drug benefits through most of the major pharmacies. You can purchase a 30-day supply for generic, preferred brand name drugs or non-preferred brand name drugs at varying co-payment amounts. The prescription drug benefit also offers a mail-order program for a 90-day supply of maintenance drugs at only twice the cost of a 30-day supply.

Vision Care

When you enroll in a medical plan, you will automatically receive vision care services. All medical plans offer the same vision care benefits. Vision Care provides one routine eye examination every 12 months by an optometrist or an ophthalmologist and discounts on lenses and frames. Although you may visit any vision doctor, your benefits may be greater if you select an in-network provider.

Dental Insurance

When you enroll in a medical plan, your dental coverage is also included.

Disability Insurance

Our disability plans offer income replacement to provide financial protection for you and your family if you can't work over a temporary period of time because of illness, pregnancy, or injury. Salient Surgical Technologies, Inc. provides basic Short-Term and Long-Term Disability coverage at no cost to you.

Reimbursement Accounts

Reimbursement Accounts are a great way to save by paying eligible health care and dependent care expenses with before-tax dollars. You can participate in two reimbursement accounts: the health care spending account and the dependent care spending account.

  • Health Care Spending Account - You can set aside up to $3,000 on a pre-tax basis each calendar year in a health care spending account to pay for eligible unreimbursed health care expenses, including deductibles, medical and dental co-payments, and vision care.
  • Dependent Care Spending Account - You can set aside up to $5,000 on a before-tax basis each calendar year in a dependent care spending account to pay for eligible dependent care expenses, including childcare and eldercare services.

Life Insurance

Salient Surgical Technologies, Inc. is committed to ensuring that your family members are provided with financial security in the event of death. Our Basic Life and AD&D Insurance are considered a core benefit and paid at 100% by the company. Because each of us has different insurance needs - you will also have the choice of purchasing additional life insurance for both you and your family members with the Portable Term Family Life Insurance Program.

  • Basic Employee Life and AD&D - Basic employee life and AD&D insurance in the amount of 1 times annual earnings up to $100,000 at no cost to you.
  • Employee/Family Life and AD&D - You can choose additional life insurance coverage for yourself and your family at a group rate.

 

Short-Term Disability Plans

  • Basic Value Plan - Pays a benefit equal to 66.67% of your average weekly salary. The weekly benefit maximum is $1,000 and is payable through 26 weeks of disability. Salient Surgical Technologies , Inc. pays the full cost of the basic plan.
  • Premier Plan - Pays a benefit equal to 70% of your average weekly salary. The weekly benefit maximum is $1,500 and is payable through 26 weeks of disability. You pay only the difference between the Company-provided Basic Value Plan and the cost of the Premier Plan.

Long-Term Disability Plans

  • Basic Value Plan - After 26 weeks of disability, the basic plan pays a benefit equal to 60% of your average monthly salary. The monthly benefit maximum is $10,000. The benefit is payable up to the duration of your disability to your normal Social Security retirement age. Salient Surgical Technologies, Inc. pays the full cost of the basic plan.
  • Premier Plan - After 26 weeks of disability, the enhanced plan pays a benefit equal to 66.67% of your average monthly salary. The monthly benefit maximum is $15,000. The benefit is payable up to the duration of your disability to your normal Social Security retirement age. You pay only the difference between the Company-provided basic value plan and the cost of the premier plan.
